Important: We Do Not Provide Medical or Personal Care Services
Bridge to New Beginnings is independent living housing only. We do not provide, manage, or coordinate medical care, nursing services, medication management, personal care assistance, or any form of supervised care. Residents who require such services must arrange them independently or through their case manager.
Arranging Outside Services
Residents may receive services from outside providers. Here’s how it works.
Medical Care
Home health aides, visiting nurses, physical therapy, and other medical services.
Transportation
Medical transport, ride services, and mobility assistance programs.
Social Services
Case management, counseling, and community support programs.
Benefits Coordination
Assistance with SSI, SSDI, VA benefits, and other entitlements.
How Outside Services Work
Resident-Arranged: Residents are responsible for arranging their own outside services. This includes scheduling, coordinating, and communicating with providers.
Provider Access: Outside service providers may visit our homes to provide care to residents. We ask that visits be scheduled during appropriate hours and that providers respect other residents’ privacy.
No Liability: Bridge to New Beginnings does not manage, supervise, or assume responsibility for services provided by outside agencies. Residents and their providers are responsible for their own arrangements.
Case Manager Coordination: If you have a case manager, they can help coordinate services before and after move-in. We’re happy to communicate with case managers as needed.
For Case Managers & Coordinators
We welcome partnerships with case managers, social workers, VA coordinators, and hospital discharge planners. Our team is happy to communicate about resident needs and housing arrangements.
While we don't coordinate services directly, we can provide information about our housing environment and work with you to ensure a smooth transition for your clients.
